Network Health Partners With Health Integrated for Coaching
Medford, Mass.-based health plan Network Health will use the Synergy Targeted Population Management program from Tampa, Fla.-based Health Integrated to offer its members health coaching services.
Synergy will help Network Health manage the care of its members who suffer from conditions such as diabetes, chronic obstructive pulmonary disease, congestive heart failure, asthma, depression and anxiety, the companies said in a release. It works by applying proprietary algorithms, sophisticated analytics and predictive modeling to identify the targeted population based on risk, complexity and opportunity, according to Health Integrated's Web site.
"As a health management solutions partner, Health Integrated offers an innovative, whole-person approach to improving our members' health," Mary Jane McKendry, senior director of clinical affairs operations for Network Health, says in a statement. "We are pleased to partner with Health Integrated to enhance our disease management strategies and achieve our aggressive clinical outcomes, quality measures, and efficiency goals."
